More care closer to home
…Increasing the opportunity to have tests, treatments and appointments carried out in a local or community setting, such as your local GP practice, rather than having to travel to a hospital
Bigger, better, safer together
…Properly coordinating services in many different areas. For example, services for frail older people and those with long-term conditions so that easier access to specialist advice is available and fewer hospital stays are needed
…And combining expertise across our hospitals to make sure that, in an emergency, you have access to a specialist opinion 24 hours a day, seven days a week.
The areas of focus
So far senior clinicians and managers in our hospitals have highlighted six areas where they believe improvements could be made in terms of quality, safety and affordability. The areas are:
Services for people with long term conditions, such as diabetes
Planned care including general surgery
Emergency services such as accident and emergency.
We now want to hear from more people – our patients, local residents and all our staff groups – on how they feel these services could change.
